Full-time Faculty are eligible for medical,
dental, vision, flexible spending accounts, employee assistance
plan & life insurance benefits effective the first of the month
following hire date. Eligibility for long term disability
benefits is the first of the month following one year of
employment.

Quinnipiac University’s “Employee Benefits Central” (EBC) web
portal is your online home base for reviewing and accessing
information about the University’s health and welfare benefits.
Everything you need to know about the valuable health and
welfare benefits the University provides can be found by
accessing the ‘EBC’ website. This includes comprehensive
information about your medical, dental, vision, life insurance,
long term disability, flexible spending accounts and employee
assistance program (EAP)including all plan documents, rates,
forms, and contact information for each carrier. In addition to
the wealth of information on this site, you can also access the
Health and Welfare Benefits Help Desk ‘online benefit inquiry
form’ for any questions you have pertaining to coverage, claims,
life status changes, etc. The Benefits Help Desk keeps all
inquiries and personal data completely confidential and complies
with all HIPAA regulations. Spouses and dependents are invited
to use this resource as well as employees. If you prefer to
call, the help desk number is 877-373-6535. Spanish speaking
representatives are available as well as additional language

Quinnipiac Benefit Plan Coverage:
You are required to complete and return the following forms to
Human Resources even if you elect not to enroll in Quinnipiac's
Health Insurance Plans:

For those plans you wish to enroll in, please complete and
submit selected enrollment forms prior to the effective date of
your insurance:
Medical/Dental/Prescription Coverage
Anthem Application (pdf) (Required if electing Anthem Medical
and/or required if electing Co-Pay or Flex Dental)
Oxford Application (pdf) (Required if electing Oxford Medical)
Info on Domestic Partner Coverage & Form (pdf) (Only required if
electing domestic partner coverage)
Vision Coverage
EyeMed Enrollment Form (pdf) (Required if electing vision
coverage)
Dependent Coverage
Employees electing dependent coverage under the University's
medical, dental or vision plans are required to verify dependent
status. Please review attached list of "Dependent Eligibility
Documents" and submit proof of eligibility with your completed
enrollment forms.

Should you elect to waive the University's medical and dental
coverage, you must complete and submit the following form:
